What is a Complex Fracture?

A complex fracture is a serious break in which the bone is shattered into several pieces, badly displaced, open through the skin, or extends into a joint. It often comes with damage to the surrounding muscles, nerves and blood vessels. Road accidents, falls from height and sports injuries are common causes, and in elderly people even a simple fall can cause one because the bones are weak.

These injuries rarely heal well with a plaster alone. They usually need precise surgical fixation so the bone can be put back in its natural shape and the patient can start moving early.

Common Types

Bone in 3 or more pieces

Comminuted Fractures

High energy injuries where the bone breaks into many fragments. They need careful reconstruction with plates, screws or nails.

Extends into a joint

Intra-articular Fractures

Breaks involving the joint surface of the knee, ankle, hip, shoulder or elbow. Exact repair is needed to prevent early arthritis.

Bone through the skin

Open (Compound) Fractures

A surgical emergency because of the high risk of infection. Needs wound cleaning and stable fixation without delay.

Failed or wrong healing

Non-union & Malunion

Fractures that did not heal or healed in a bad position. Corrected with revision fixation and bone grafting.

How Complex Fractures Are Treated

  • Emergency care, pain relief and stabilising the limb
  • X-ray and CT scan to plan the exact fixation
  • Wound cleaning and antibiotics for open fractures
  • Fixation with plates, screws, intramedullary nails or external fixators
  • Joint replacement when the joint surface cannot be saved, mainly in the elderly
  • Bone grafting for fractures that fail to heal
  • Early guided physiotherapy to regain movement and strength
